First Visit
If your family dentist feels you need endodontic or periodontal treatment, he or she may refer you to our office. Continue to take your medications as directed. When you arrive, our staff will assist you in completing our registration form. You may prefer to print the Patient Registration Form ahead of time and bring it to your appointment. Or, you may fill-out the online Patient Registration form.
Endodontic Patients
Your consultation will include a review of your health history as well as an oral evaluation that includes several diagnostic tests to help us determine your treatment needs. With advance planning most endodontic treatment can be completed the same day as the consultation; however, a complex treatment plan may require a second appointment.
Periodontal Patients
Please notify the office if you have a medical condition or concern prior to surgery (e.g. artificial heart valves or joints, heart murmurs requiring pre-medication, severe diabetes, or hypertension).
We will complete an in-depth medical and dental health history and a thorough examination to measure for bone loss, loose teeth, bite, oral cancer screening, TMJ, and other signs of periodontal disease.
Please assist us by providing the following information at the time of your consultation:
- Referral slip from family dentist and any digital images if applicable
- List of medications you are presently taking
- If you are pregnant, a letter from your obstetrician
- If you have dental insurance, your insurance card
- Your registration form if you prefer to print and complete it at home
Please Note: All patients under 18 must be accompanied by a parent or guardian at the consultation appointment.
